Job summary

Advocate Health is seeking a Home Health Physical Therapy Assistant to manage daily patient care across the York, SC region. The role requires PTA licensure, current BLS certification, and prior home health or clinical PT experience.

You will work Monday through Friday, 8:30am-5pm, with a 40-hour work week and a sign-on bonus up to $7,500 for qualified applicants. Responsibilities include data collection from home assessments, implementing therapy plans, and documenting progress.

Qualifications

• PTA licensed in good standing and able to practice
• BLS HCP certified and current
• Experience in home health or clinical PT environments preferred
• Valid driver’s license and auto insurance maintained

Responsibilities

• Manage assigned patient caseload daily with plan of care and MD orders
• Collect health data from home assessments and EMR
• Implement care programs including modalities, procedures, and home exercise
• Document plan of care addressing patient/family needs and discharge planning
• Provide patient/family education on deficits, goals, and home programs
• Communicate updates with multidisciplinary team and physician
• Ensure documentation per policy and productivity targets
