Advocate Health At Home is seeking a licensed Physical Therapist to provide home health services in the Lincoln region on a full-time basis. The role involves managing a patient caseload, coordinating care with physicians and a multidisciplinary team, and documenting progress and outcomes.

The ideal candidate will hold a DPT and PT license, with BLS/CPR certification preferred. Sign-on bonus up to $15,000 available for qualified applicants and a referral bonus of $7,500 for internal teammates.

Formación

• DPT degree required.
• Physical Therapy License required.
• Emergency/CPR/BLS certification expected.

Responsabilidades

• Manage daily home health PT caseload and ensure timely care per plan of care.
• Collect and analyze patient data from assessments and EMR.
• Coordinate with physician and multidisciplinary team to optimize outcomes.
• Maintain thorough documentation in visit notes, progress notes, and discharge summaries.
• Ensure compliance with Home Health guidelines and state regulations.
