Job summary

UnitedHealth Group’s MS HomeCare division is seeking a Physical Therapist for Home Health. You will assess patient needs, develop and adjust the plan of care, and provide interventions to improve well-being and mobility in home settings.

The role requires current state PT licensure and a valid driver’s license; CPR certification is preferred. You’ll participate in a comprehensive benefits package and contribute to high-quality patient care across communities.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within scope of practice as defined by state laws.
• Observes and reports patient responses to treatment to supervising nurse or physician.
• Treats patients to relieve pain, improve function, and maintain maximum functioning.
• Instructs patient and family about plan of care and progress.
