Job summary

TEEMA is seeking a dedicated Home Health Physical Therapist to deliver medically prescribed, goal‑oriented care in patients’ homes in West Branch, Michigan. You will assess, design individualized plans, and perform restorative therapies in collaboration with an interdisciplinary team.

You will supervise aides and assistants, educate patients and families, and ensure documentation aligns with regulatory standards. This role offers a patient-centered, fulfilling PT career in a field setting.

Qualifications

• Education: Bachelor's or Master’s degree in Physical Therapy from an accredited institution.
• Licensure: Current, unencumbered Physical Therapist license in good standing with the state Board of practice.
• Credentials: Valid driver’s license and current automobile insurance.

Responsibilities

• Perform initial and ongoing assessments, including OASIS assessments at designated time points.
• Develop and implement patient plans of care in collaboration with attending physicians.
• Deliver skilled restorative treatments (gait training, ultrasound, electrical stimulation, prosthetic training, balance exercises).
• Perform ROM testing and use modalities (moist heat, whirlpools, paraffin) as prescribed.
• Instruct and supervise home exercise programs and provide home safety guidance.
