Job summary

TEEMA Group is seeking a dedicated Home Health Physical Therapist to deliver medically prescribed, goal-oriented care within a 60-mile travel radius. You will assess, design, and implement individualized treatment plans and supervise therapy staff in a patient-centered home health setting.

The role requires a current PT license in good standing, a valid driver's license, and proficiency with Homecare Homebase (HCHB). Expect a Monday–Friday schedule with travel to patients’ homes.

Qualifications

• Education: Bachelor's or Master’s degree in Physical Therapy from an accredited institution.
• Licensure: Current, unencumbered Physical Therapist license in good standing with the state Board of practice.
• Credentials: Valid driver’s license and current automobile insurance.
• Software Proficiency: Homecare Homebase (HCHB) EMR system.
• Specialized Knowledge: PT principles for terminally ill and complex patient care.
• Patient Management: bedside care, symptom management, crisis intervention, and family intervention.
• Interpersonal & Decision-Making: excellent communication, problem-solving, and decision-making with rapport.
• Experience: 1+ year as a Physical Therapist in a multidisciplinary or home health setting.

Responsibilities

• Perform initial and ongoing comprehensive assessments including OASIS and evaluate care outcomes.
• Maintain a daily caseload of ~6 patient visits within a 60-mile radius.
• Develop and implement patient plans of care with attending physicians.
• Deliver skilled restorative treatments to reduce pain and restore/maintain function.
• Supervise Physical Therapist Assistants, CNAs, and Home Health Aides.
• Document care accurately in EMR and coordinate team communication.
• Participate in team meetings and ongoing staff education.
