Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to perform assessments and develop treatment plans to improve patients’ functional status and mobility. You will implement interventions, monitor progress, and collaborate with the health care team to adjust plans of care.

Responsibilities include evaluating patients, developing and documenting the plan of care, and ensuring care quality within state guidelines. Strong communication with patients and families is essential.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Assists physician in developing the therapy plan of care.
• Provides services within state practice scope and plan of care.
• Observes and reports patient responses to treatment and changes.
• Treats patients to relieve pain and restore function.
• Instructs patient and family on plan of care and progress.
• Completes progress notes for each visit and submits per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
