Job summary

Vitality at Home is seeking a licensed Physical Therapist to provide skilled PT services in patients' homes in Minnesota. You will assess mobility, design individualized care plans, and implement evidence-based interventions to improve independence and safety.

You will collaborate with an interdisciplinary team and educate patients and caregivers on maintaining function. A valid Minnesota PT license, CPR/BLS, and a current driver's license are required.

Qualifications

• Graduate of an accredited Physical Therapy Doctorate program.
• Licensed to practice as a Physical Therapist in Minnesota.
• CPR certification (BLS) required.
• Minimum of one year experience in physical therapy (home health or geriatric preferred).
• Valid driver’s license and reliable transportation.
