AccentCare is seeking a Home Health Physical Therapist to deliver skilled in-home care across the Los Angeles area. You’ll evaluate mobility, design personalized treatment plans, and guide patients through exercises to regain strength and independence.

This full-time role runs Monday–Friday, 8 am–5 pm, with a $10k bonus and annual pay range of $124,594 to $152,282 plus mileage. Benefits include medical, dental, PTO, 401(k) matching, and more.

Qualifikationen

• Community and/or home health experience preferred.
• Master's degree in physical therapy approved by a CAPTE-accredited program.
• Current physical therapist license within the state of operation.
• Valid CPR certification required.
• Must be a licensed driver who can travel to all locations.

Aufgaben

• Assess mobility and functional limitations to establish a home rehabilitation plan.
• Create personalized treatment plans to improve strength, balance and independence.
• Deliver one-on-one hands-on therapy to improve mobility and function.
• Educate patients and families on home exercise programs and safety techniques.
• Monitor progress, adjust plans, and coordinate with the care team.
