Job summary

VitalCaring is seeking a Home Health Physical Therapist in Georgetown, Texas. This PRN role focuses on delivering patient-centered care in patients’ homes, promoting independence and mobility while working with a dedicated care team.

The role offers autonomy, clinical excellence, and opportunities to impact patients and families through hands-on therapy, education, and coordinated care. A valid Texas PT license and CPR are required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• CPR certification is required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises and safety.
• Monitor progress, adjust plans, and support safe transitions of care.
• Document care and progress in the EMR; participate in team meetings.
• Supervise PTAs in accordance with state guidelines; mentor caregivers.
