Job summary

UnitedHealth Group's Caretenders division seeks a Licensed Physical Therapist to assess patient needs in home health, develop and adjust treatment plans, and collaborate with the care team to improve mobility and well-being.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient's response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Informs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment and submits progress notes.
• Documents evaluations and develops the PT plan of care within state guidelines.
