Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health in Florida to assess patients’ mobility and functional status, develop individualized care plans, and deliver interventions to improve endurance and pain management.

You will collaborate with physicians and the care team, document progress, and ensure care complies with state guidelines. The role includes a comprehensive benefits package and growth opportunities within the Optum family.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and PT needs; assists in developing the therapy plan of care.
• Provides PT services within scope of practice and plan of care, coordinating with the health care team.
• Observes and reports patient responses to treatment and changes in condition to the supervising nurse/physician.
• Delivers treatments to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and care team about plan of care and progress toward goals.
• Visits patients as per Plan of Treatment; completes progress notes per policy.
• Completes PT evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care; documents productivity.
