Job summary

Responsibilities include evaluating functional status, delivering interventions, documenting progress, and ensuring care plans meet state guidelines. Strong communication with patients and families is essential in this role.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Provides services within the scope of practice and plans of care.
• Observes and reports patient response to treatment and condition changes.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family regarding plan of care and progress.
• Visits patients according to Plan of Treatment and documents progress.
• Completes patient evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function effectively in diverse home settings.
