Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to assess and evaluate patient care needs related to functional status and mobility. The PT develops a treatment plan, implements interventions, and evaluates progress to improve patient well‑being.

Responsibilities include evaluating functional status, collaborating with physicians, and maintaining documentation and plan of care within state guidelines. Weekend rotation and some holiday coverage may be required.

Qualifications

• Licensed in Physical Therapy in the state of residence.
• Current CPR certification.
• Current driver's license, vehicle insurance, access to a dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient's functional status and develops the therapy plan of care in consultation with the physician.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
