Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to perform assessments, evaluate care needs, and develop treatment plans. You will deliver interventions to improve mobility and function, and monitor progress with a goal of maximizing patient well-being while collaborating with physicians and the health-care team.

Responsibilities include evaluating functional status, planning care within state guidelines, documenting progress, and educating patients and families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patients' functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and plan of care; coordinates with health-care team.
• Observes patient responses and reports changes to supervising nurse/physician.
• Treats patients to relieve pain and restore function; educates patient and family about plan of care.
