Job summary

Caretenders Home Health, part of the LHC Group family, seeks a Physical Therapist to assess and treat patients in home health settings under Optum. You will determine treatment plans, perform interventions to improve well-being, and evaluate patient progress with the health care team.

Responsibilities include conducting functional assessments, developing plans of care, documenting progress, and coordinating with physicians and nurses to deliver high-quality home-based therapy services.

Qualifications

• Evaluates patient functional status and PT needs.
• Involves assessment, planning, and documentation per state guidelines.
• Communicates with care team and patients/families about plan of care.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Provides services within scope of practice per plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function in diverse home settings without discrimination.
