Job summary

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a part-time opportunity performing home health visits in Raleigh, North Carolina. This is a vital role providing rehabilitative care to adult and geriatric clients in Wake, Durham, and Johnston counties.

Qualifications

• One year prior clinical experience as a licensed Physical Therapist required.
• Must have home health/OASIS experience.
• Must be willing to cover all counties including Wake, Durham, and Johnston.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements.
• Develop and implement individualized care plans.
• Accurately document observations using a tablet.
