AccentCare in El Centro, CA is seeking a Physical Therapist for Home Health to evaluate and treat patients in collaboration with an interdisciplinary team. The role covers El Centro and surrounding areas; schedule Monday through Friday; full-time, on-site, with local travel.

You will document care per physician orders, develop individualized treatment plans, educate patients and families, and participate in in-services.

Qualifikationen

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Currently licensed to practice as a physical therapist within the state of operation.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver who can travel to all business locations.

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic material.
• Informs physician of assessment results and recommended interventions before initiating care.
• Develops and documents treatment plans and priorities.
• Provides organized and effective patient treatment including education and counseling.
• Presents patient information to physicians and keeps team informed of status and plan.
• Delivers care within agency and payer guidelines and tracks goals with measurable outcomes.
• Meets productivity standards and uses time efficiently.
