Job summary

Pathways Healthcare is seeking a Physical Therapist to join our Home Health team, delivering patient-centered rehabilitation across Methuen and surrounding areas. You will assess, plan, and implement therapy programs in patients’ homes, ensuring high-quality care aligned with physician plans of care.

We value clear communication, clinical judgment, and commitment to improving patients' independence. A valid driver’s license and reliable vehicle are required.

Qualifications

• Degree as a physical therapist approved by an accredited organization.
• Minimum of two years of relevant experience as a physical therapist (preferred).
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Provide physical therapy services per physician's plan of care and state law.
• Develop and implement individualized therapy programs.
• Instruct patients and families in use of therapeutic devices and appliances.
• Attend discharge planning and coordinate referrals to home health services as needed.
• Prepare clinical progress summaries and document outcomes.
