Job summary

Pathways Healthcare is seeking a dedicated Physical Therapist to serve Stoneham and surrounding areas. You will deliver therapy per a physician's plan of care, assess mobility and functional status, and plan rehabilitation in patients' homes.

Require degree in physical therapy, MA license, 2+ years of PT experience (home health preferred), and BLS CPR. Valid driver's license and reliable vehicle required. Full-time with a supportive team.

Qualifications

• Degree from an accredited Physical Therapy program.
• Active Massachusetts Physical Therapist license.
• Minimum 2 years of PT experience; home health/community experience preferred.
• Current BLS CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Deliver physical therapy services per physician's plan of care.
• Assess mobility, strength, gait, and range of motion; evaluate home environment.
• Provide instruction on use and care of therapeutic devices.
• Report patient responses and changes in condition to the physician.
• Collaborate with other healthcare professionals and family members.
