The role emphasizes physician-guided care and patient-centered outcomes. The ideal candidate has a valid MA license, 2+ years of PT experience (home health preferred), and possesses strong communication and organizational skills.

Qualifications

• Degree from an accredited Physical Therapy program.
• Valid MA Physical Therapy license required.
• Minimum 2 years of clinical experience; home health/community care preferred.

Responsibilities

• Deliver physical therapy services per physician plan of care and state regulations.
• Assess therapeutic needs, mobility, gait, and ROM; evaluate home environment for rehab suitability.
• Provide patient and caregiver instruction on therapies and appliance use; document progress.
