Job summary

Pathways Healthcare, Llc is seeking a licensed Physical Therapist to join our home health team serving Stoneham and surrounding areas. This full-time role operates Monday–Friday, 8:30 am–5:00 pm, with emphasis on patient-centered care in patients' homes.

Ideal candidates have at least two years of PT experience (home health preferred), strong communication skills, and a current BLS CPR certification. A valid MA driver's license and reliable vehicle are required.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Hold a valid Massachusetts Physical Therapist license.
• Two years of relevant experience, with home health/community setting preferred.

Responsibilities

• Deliver physical therapy services in accordance with a physician's plan of care and state PT Practice Act.
• Assess and evaluate therapeutic, rehabilitative, and functional status, including home environment.
• Conduct strength, mobility, gait and range of motion assessments to identify rehabilitation potential.
• Administer PT treatments and provide instruction on use and care of devices.
• Document patient progress and communicate changes to the physician.
