Job summary

Pathways Healthcare is seeking a Physical Therapist - Home Health to provide patient-centered care in Lawrence, MA, with service coverage for nearby towns.

You will evaluate patients, develop plans of care, coordinate with families and other clinicians, and support transitions home. A minimum of two years' PT experience and a valid driver's license are preferred; BLS CPR certification is required.

Qualifications

• Hold a Physical Therapist degree approved by an accredited organization.
• Two years of PT experience preferred, community/home health favored.
• Excellent verbal and written communication and strong organizational abilities.
• Maintain current BLS CPR certification.
• Valid driver's license and own reliable vehicle.

Responsibilities

• Provide PT services per physician plan of care and state law.
• Assess rehabilitation potential and patient functional status in home setting.
• Develop individualized PT programs and educate family/caregivers.
• Participate in discharge planning and coordinate with other home health services.
• Submit progress summaries and contribute to peer consultation as needed.

Job Description:

• Role: Physical Therapist - Home Health
• Rate: Per Visit
• Main Office Location: Lawrence, MA
• Service Area: Methuen, Lawrence, Haverhill, Andover, North Andover, Tewksbury areas.

Qualifications:

• Possess a degree as a physical therapist approved by an accredited organization.
• A minimum of two years of relevant experience as a physical therapist (preferred), with priority given to those with community/home health experience is desirable.
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Provide physical therapy services to patients in accordance with a written physician's plan of care and the state Physical Therapy Practice Act. This includes assessing and evaluating therapeutic/rehabilitative/functional status, evaluating the home environment, and determining the potential for rehabilitation, including muscle strength, mobility, gait, and range of motion.
• Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient's care. Actively participate in case conferences to ensure a comprehensive and coordinated treatment approach.
• Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices. Additionally, provide in-service education programs for nursing organization personnel as needed.
• Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
• Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
• Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.
