Job summary

Pathways Healthcare in Massachusetts is seeking a Physical Therapist - Home Health to assess and treat patients in their homes across Methuen, Lawrence, and surrounding communities. You will develop individualized plans of care and collaborate with families and care teams, guided by physician orders and state practice acts.

This role emphasizes reducing documentation time through AI-assisted tools while delivering high-quality, patient-centered care.

Qualifications

• Possess a degree as a physical therapist approved by an accredited organization.
• A minimum of two years of relevant experience as a physical therapist (preferred).
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Provide physical therapy services to patients per a physician's plan of care and state guidelines; assess functional status, mobility, gait, and range of motion.
• Develop individualized therapy programs and educate family/caregivers involved in care; participate in case conferences.
• Instruct patients in use and care of therapeutic appliances and prosthetic devices when needed; provide in-service education as required.
• Participate in discharge planning and refer to other home health services as needed.
• Prepare clinical progress summaries and engage in peer consultation to enhance care quality.
