Job summary

Alt is seeking a Physical Therapist for home health in Texas. This field-based, full-time role involves owning caseload, in-home assessments, gait/balance training, and safe transfers. You will coordinate with the care team, supervise PTAs as needed, and coach patients with practical home programs.

Expect a productivity-based pay model at 75.00 per point with about 30 points per week, plus scheduling support and technology to streamline documentation in HCHB.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers and functional mobility.
• Set measurable goals, track progress, and adjust treatment as needed.
• Drive multidisciplinary coordination and keep the care team aligned.
• Manage treatment/outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
