Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to join our field-based home health team in Texas. You will own the caseload, deliver skilled PT visits, and drive outcomes with a plan of care focused on gait, balance, transfers, and home safety.

You will coordinate with a multidisciplinary team, supervise PTA where appropriate, and coach patients and caregivers with practical home programs. Compensation includes per-visit rates and an annual range of $100,000–$130,000, with travel

Qualifications

• Active Texas PT license in good standing.
• Strong independent field work with clinical judgment.
• Excellent communication and thorough documentation.
• Willingness to travel across assigned territory.
• Competency in core home health PT scope (gait, transfers, home safety).

Responsibilities

• Own the caseload as case manager for the patient’s therapy plan and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Develop and execute a plan of care focusing on gait/balance, transfers, strengthening, mobility, and home safety.
• Set measurable goals, track progress, and adjust treatment as needed.
• Coordinate with the care team and supervise PTA where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB.
