Job summary

AccentCare, Inc. is seeking a Physical Therapist for Home Health in San Antonio, TX to join an interdisciplinary team. This role involves evaluations, treatments, and education, with a caseload managed in line with agency productivity standards.

Qualified candidates will hold a CAPTE-accredited PT degree, be licensed in Texas, have CPR, and be able to travel to all locations. This position offers comprehensive benefits and growth opportunities.

Qualifications

• Master's degree in physical therapy approved by CAPTE accredited physical therapist education program.
• Minimum nine months experience as a physical therapist.
• Currently licensed to practice as a physical therapist within the state of operation.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations and selects diagnostic material to assess all aspects of patient care.
• Informs the physician of assessment results and recommended clinical interventions before initiating care.
• Develops and documents treatment plans with appropriate intervention approaches and priorities.
• Provides organized and efficient patient treatment including education and counseling.
• Presents pertinent patient information to physicians and keeps team informed of status.
• Delivers treatment within agency and payer parameters and patient needs.
• Sets objective, measurable goals in collaboration with the team and patient.
• Meets productivity standards and uses work hours efficiently.
