Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health services. The role focuses on assessing patient needs, developing and implementing plans of care, and coordinating with physicians and nurses to optimize patient outcomes.

Responsibilities include evaluating functional status, treating to relieve pain and restore mobility, and educating patients and families. State licensure and reliable transportation are required, with CPR preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient’s functional status and needs and assists the physician in developing the therapy plan.
• Provides PT services within scope of practice in accordance with the plan of care and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, limitations and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
