Job summary

Optum's IL Home Health unit is seeking a dedicated Physical Therapist to assess patients in their homes, evaluate mobility, endurance and pain, and help plan targeted treatment to improve well-being.

You will implement therapy plans, document progress, coordinate with physicians and nurses, and ensure care stays within state guidelines. A current PT license, CPR certification, and reliable transportation are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license with access to a vehicle.
• Ability to provide care in diverse home environments.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing PT, per plan of care, with health care team.
• Observes and reports patient response to treatment and changes to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient/family about plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment, completes progress notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
