Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.

Responsibilities

• Assess and evaluate patient mobility, endurance, pain, and other conditions.
• Develop and implement PT plan of care in collaboration with care team.
• Document patient progress and adjust plans per state guidelines.
• Communicate with patients, families, and care team about plan of care and goals.
• Provide timely progress notes and outcomes of evaluations.
