Job summary

ABL Health Care in Vacaville is seeking a full-time Physical Therapist (PT) to join our Home Health team. You will evaluate patients, develop and implement care plans, and coordinate with our Clinical Director and staff to deliver high-quality care in patients' homes.

The role requires a California PT license, at least 1 year in home health or hospice, a valid CA driver's license, and willingness to travel within a 15–25 mile radius.

Qualifications

• Current PT license to practice in California (required).
• Experience in Home Health or Hospice care (1 year required).
• Current CA Driver's License.

Responsibilities

• Provide skilled intermittent home care and educate patients/caregivers to promote maximal functional status.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to physicians or nursing leadership.
• Develop and revise the patient's plan of care (POC).
• Maintain accurate clinical records and documentation per policy.
