Job summary

ABL Health Care is seeking a dedicated Full Time Physical Therapist (PT) to join our Home Health team in Pacifica. You will evaluate patients in their homes, develop and implement care plans, and monitor progress alongside our Clinical Director and staff.

The ideal candidate holds a California PT license, has at least 1 year of Home Health or Hospice experience, a valid CA driver's license, and is willing to travel within a 15–25 mile radius.

Qualifications

• Current California PT license to practice is required.
• Experience in Home Health or Hospice care (1 year) is required.
• Valid California driver's license; must be able to travel regularly within a 15–25 mile radius.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care in patients' homes.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions and report changes to the relevant clinicians.
• Develop and revise the patient's plan of care (POC) and address issues identified in the POC.
• Educate patients and caregivers and communicate with involved disciplines.
• Maintain accurate clinical records using company devices and comply with policies.
