Job summary

ABL Health Care LLC is seeking a dedicated Physical Therapist (PT) for our Home Health team in Vacaville. This full-time role involves evaluating patients, creating and implementing care plans, and tracking progress to uphold high standards of care.

You will work with the Clinical Director and team, providing consistent support in the field. The ideal candidate will have a California PT license, at least 1 year in home health or hospice, and a valid CA driver’s license.

Qualifications

• Current California PT license required.
• 1 year of Home Health or Hospice care experience preferred.
• Valid California driver's license.
• Ability to travel within a 15–25 mile radius on a regular basis.

Responsibilities

• Provide skilled intermittent care to patients in the home.
• Evaluate and re-evaluate patient status; update care plans.
• Administer treatments and monitor patient conditions; report changes to clinician.
• Develop, implement and revise the patient's plan of care (POC).
• Educate patient and caregiver on condition and care needs.
• Maintain accurate clinical records using company devices; ensure adherence to policies.
