Job summary

ABL Health Care LLC is seeking a full-time Physical Therapist Assistant (PTA) for home health in Vacaville. The PTA will evaluate patients’ physical health, develop individualized treatment plans, and enhance mobility and quality of life while collaborating with the Clinical Director and team.

Responsibilities include implementing plans, delivering treatments, documenting care, and maintaining compliance with policies.

Qualifications

• CA PTA license required in California.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
