Job summary

PRIDE Health is seeking a Local Contract Home Health Physical Therapist to support patients in Highlands and Hardee Counties, Sebring, Florida. The PT will deliver skilled home-based physical therapy, evaluate progress, and implement care plans to maximize mobility and independence. Schedule is Monday–Friday, 8:00 a.m.

– 4:30 p.m. with 40 hours per week, for a 14-week assignment. Mileage reimbursement is provided; travel between patient visits is expected; OASIS experience and HCHB proficiency

Qualifications

• Active Florida Physical Therapist license.
• Minimum 1 year of Home Health experience.
• Experience completing OASIS assessments.
• Proficiency with Homecare Homebase (HCHB).

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Complete OASIS assessments, reassessments, recertifications, and discharge evaluations as required.
• Develop and implement individualized Plans of Care based on physician orders and patient goals.
• Provide therapeutic interventions to improve strength, mobility, balance, and independence.
• Monitor patient progress and modify treatment plans to achieve outcomes.
• Educate patients and caregivers on exercises, safety, and disease management.
• Collaborate with physicians and interdisciplinary team members.
• Maintain accurate documentation in Homecare Homebase (HCHB).
• Meet productivity expectations while delivering patient-centered care.
