Job summary

Palmshomecare in Florida is seeking a dedicated Home Health Physical Therapist (PT) to provide in-home therapy, helping patients regain mobility and independence after illness, injury, or surgery.

You will perform evaluations, develop individualized plans, deliver therapeutic exercises and education, document care, and collaborate with care teams while adhering to Medicare guidelines. License, CPR, and reliable transportation are required.

Qualifications

• Current Physical Therapist license
• Graduate of accredited PT program
• 1–2 years of experience preferred in Home Health
• Strong communication skills
• Ability to work independently
• Possesses and maintains current CPR certification
• Valid driver’s license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser

Responsibilities

• Perform initial evaluations and develop individualized treatment plans
• Provide therapeutic exercises, manual therapy, and mobility training
• Assess patient progress and adjust care plans accordingly
• Educate patients and caregivers on safety and exercises
• Maintain timely and accurate documentation
• Collaborate with interdisciplinary care teams
• Follow home health and Medicare guidelines
