Job summary

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for assignments in Sebring, Florida. You’ll evaluate and treat patients in their homes, perform OASIS visits, and document progress across Highlands and Hardee Counties.

The role offers 40 hours per week, Monday–Friday, 8:00 AM to 4:30 PM, with mileage reimbursement and a comprehensive benefits package designed for travelers. Licensure and BLS requirements apply.

Qualifications

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• At least 1 year home health experience
• HCHB experience
• Current/Active Physical Therapy License

Responsibilities

• Provide Physical Therapy services in patient homes, including OASIS visits
• Assess and treat patients in home health settings, documenting progress
• Maintain productivity expectations and coordinate with care teams
