Job summary

Palms Home Care seeks a skilled Home Health Physical Therapist to provide patient-centered therapy in patients' homes across Florida. You will evaluate, design and implement personalized treatment plans to improve mobility and independence after illness, injury, or surgery.

Qualifications include a valid PT license, CPR, and 1–2 years in home health preferred. Strong communication, independence, and reliability are essential, with travel to patient homes required.

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1–2 years of home health experience preferred.
• Strong communication and independent work skills.
• CPR certification current.
• Valid driver's license and reliable transportation.
• Knowledge of OASIS and EMR systems such as Kinnser.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans
• Provide therapeutic exercises, manual therapy, and mobility training
• Assess patient progress and adjust care plans accordingly
• Educate patients and caregivers on safety and exercises
• Maintain timely and accurate documentation
• Collaborate with interdisciplinary care teams
• Follow home health and Medicare guidelines
