Job summary

Pride Health is seeking a licensed Physical Therapist to provide home-based care in Sebring, FL. Schedule is Monday–Friday, 8:00 AM–4:30 PM, with 13-week local contract and pay range $52–$57/hour.

Responsibilities include comprehensive PT evaluations, OASIS duties, and plan-of-care development for Highlands and Hardee counties. Requirements include an active Florida PT license, 1+ year home health experience, and proficiency with OASIS and HCHB.

Qualifications

Active and unrestricted Florida Physical Therapist license.
Minimum 1 year of Home Health experience.
Experience completing OASIS assessments.
Proficiency with Homecare Homebase (HCHB).

Responsibilities

Perform comprehensive PT evaluations in the home setting.
Complete OASIS assessments, reassessments, and discharge evaluations as needed.
Develop and implement Plans of Care based on physician orders and patient goals.
Provide therapies to improve strength, mobility, balance and endurance.
Monitor progress and adjust treatment plans for outcomes.
Educate patients and families on exercises, safety, and disease management.
Recommend assistive devices and adaptive equipment.
Collaborate with physicians and the healthcare team.
Maintain accurate documentation in HCHB and meet productivity targets.
