Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes, evaluating mobility and rehabilitation potential, and developing individualized care plans with physicians and the care team.

You will supervise PTAs as allowed by state guidelines, educate patients and caregivers, and collaborate with nurses and physicians to ensure coordinated care, documentation, and timely progress notes.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Deliver patient-centered care by evaluating and assessing patients’ functional status, mobility, strength and rehabilitation potential
• Develop and implement individualized care plans with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance and independence
• Educate patients and caregivers on treatment plans, exercises and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner with nurses, therapists and physicians to ensure coordinated care
• Document patient care and progress in the EMR
• Participate in case conferences and team meetings
