VitalCaring, a national home health and hospice provider, seeks a dedicated Physical Therapist to deliver patient-centered care in patients' homes. You will assess function, develop individualized plans, and collaborate with physicians and the care team to advance mobility and independence.

This role emphasizes autonomy, ongoing education, and strong relationships with patients and families, with opportunities to mentor PTAs and contribute to discharge planning.

Formación

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

Responsabilidades

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
