Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team in Haltom City, TX. This role focuses on evaluating patients, creating individualized treatment plans, and delivering evidence-based care in patients' homes.

You will collaborate with an interdisciplinary team, document progress using digital platforms, and educate patients and caregivers on home exercise programs and safety. A TX license and clinical experience in home health are required or preferred.

Qualifications

• Active Texas Physical Therapist license required or eligible.
• Professional liability insurance coverage specific to TX license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to individual patients.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapies including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Document evaluations, treatments, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapy professionals.
• Conduct home safety assessments and minimize fall risk.
