PARS Therapy in Lake Jackson, TX is seeking a compassionate Physical Therapist to join our home health team. This role helps patients regain movement and reduce pain, delivering evidence-based care in the comfort of their homes.

You will evaluate, plan, and implement personalized treatments, educate families, and collaborate with an interdisciplinary team to improve mobility and safety in daily life.

Qualifications

• Active Physical Therapist license in Texas (TX).
• Professional liability insurance coverage for TX license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to varied patients.
• Up-to-date knowledge of evidence-based treatments and best practices.
• Excellent communication skills and professional demeanor.

Responsibilities

• Evaluate patients’ abilities and functional limits in home health.
• Create and apply individualized treatment plans to restore function.
• Deliver therapeutic interventions: exercise, gait, manual therapy, balance.
• Educate patients and caregivers on home exercise and safety.
• Monitor progress and adjust plans to meet goals.
• Document evaluations and sessions using digital platforms.
• Collaborate with physicians, nurses, and therapists.
• Conduct home safety assessments to prevent falls.
• Provide compassionate care to empower independence.
