Job summary

UnitedHealth Group in the United States is seeking a Physical Therapist to provide home health services. You will assess patients, determine treatment plans, and implement interventions to improve mobility, endurance, and overall well-being in a home setting.

Responsibilities include conducting evaluations, documenting progress, coordinating with the health care team, and educating patients and families on care plans.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance; access to dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs as part of the plan of care.
• Provides services within the scope of practice and coordinates with the health care team.
• Observes and reports patient response to treatment and progress to supervisor/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family about the plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes, goals, and frequency of care.
