Qualifications

• Active TX Physical Therapist license required.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy.
• Home health experience preferred.

Responsibilities

• Evaluate patients’ physical abilities and limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, improve mobility and reduce pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatments, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care while promoting independence.

Education
