Job summary

PARS Therapy seeks a compassionate Physical Therapist (PT) to join our home health team in Cleburne, TX. You will evaluate patients at home, design individualized treatment plans, and deliver evidence-based therapy aimed at restoring function and reducing pain. Qualifications

• Active TX Physical Therapist license required.
• Professional liability insurance coverage for TX PT license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patient needs.
• Knowledge of evidence-based treatment techniques and best practices.
• Excellent communication and a professional demeanor while educating patients.

Responsibilities

• Evaluate patients in a home health setting and determine functional limitations.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, sessions, and progress using digital platforms.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care while promoting patient independence.
