Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Bay City, TX. This role focuses on evaluating patients at home, creating personalized plans, and delivering interventions to restore mobility and reduce pain.

Applicants must hold a Texas PT license, have professional liability insurance, and possess a graduate degree in physical therapy. Home health experience is preferred; collaboration with a multidisciplinary team is essential.

Qualifications

• Active Texas Physical Therapist license.
• Professional liability insurance for TX license.
• Graduate degree or higher in Physical Therapy.
• Home health or rehabilitative experience preferred.
• Evidence-based treatment knowledge and best practices.

Responsibilities

• Evaluate patients’ abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions and progress notes in digital systems.
• Collaborate with physicians, nurses and occupational therapists.
• Conduct home safety assessments and recommend fall prevention strategies.
• Deliver compassionate care while empowering patients toward independence.
