Responsabilidades

• Make visits to patients in their homes in Cambridge, MA.
• Perform mobility and strength assessments and gait/balance evaluations.
• Develop and implement individualized therapy plans including exercises and training.
• Update and revise care plans in coordination with the care team.
• Educate patients and families on rehabilitative care and safety at home.
• Document observations and interventions clearly using electronic records.
