Responsibilities

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements including mobility, strength, and balance.
• Develop and implement individualized care plans with manual therapy, gait training and balance interventions.
• Continually assess and revise the PT care plan and coordinate with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote independence.
• Accurately document observations, interventions and evaluations using a touch pad tablet.
