Qualifications
- Massachusetts Physical Therapist license required.
Responsibilities
- Make visits to clients in Senior Living Communities.
- Perform mobility and strength assessments and document findings.
- Develop and implement individualized care plans.
- Assess and revise care plans with nursing staff as needed.
- Educate clients and families on rehabilitative care and independence.
- Accurately document observations and interventions using a tablet.
