Job summary

BAYADA Home Health Care is seeking a Full-Time Home Health Physical Therapist to provide rehabilitative care to adult and geriatric patients in Worcester, Auburn, and surrounding towns. The ideal candidate will have a current license in Massachusetts and be proficient in client education and care plan development. Qualifications

• Current license as a Physical Therapist in Massachusetts is required.
• Experience in home health is preferred but not required.
• Understanding of diagnostic tests and therapeutic exercises.

Responsibilities

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements.
• Develop and implement individualized care plans.
• Continually assess and revise the Physical Therapy care plan.
• Educate clients and family members on rehabilitative care.
• Document observations and interventions accurately.
